This isn't most stylists' booking form, it's a diagnostic intake. Hair color is chemistry. Your history is data. The more I know before you sit in my chair, the safer I process, the better I formulate, the longer your results hold. Think dermatologist intake, not shampoo-bowl chitchat. 25+ years of corrective color, gray transitions, and advanced lightening went into knowing what to ask. Most consultations skip 90% of this. This doesn't.
Be honest about box dye, henna, or beauty-supply purchases, even years ago. No judgment, but henna and metallic salts can react violently with lightener, and box dye lifts unpredictably. I need to know to keep your hair safe.
What it covers: contact basics, current hair state, color history, goals, and photos.
Fill it out thoroughly, upload clear photos, be honest, even the box dye, even the Sun-In, even the thing your friend did. Your hair is worth it. I personally review every submission. I read your intake, look at your photos, and respond by email with next steps to get you in the chair.
